Why are we offering this service?

Swarga Foundation’s mission has been to improve the lives of individuals fighting neurological disorders. We also believe that this service will improve the quality of the lives of individuals dealing with pain and needing rehabilitation. We also benefit from the satisfaction that a person in need has been relieved of pain and / or rehabilitated.

Appointment Process

Complete the request form and we will call you in 24 hours to confirm date and time of your appointment. Alternatively, call us at 73971-65333 between 9am to 5pm, Monday through Saturday to request appointment

Cancellation of Appointment

Please call 73971-65333 as soon as you know / decide that you will not be able to attend or be on time for the appointment. We have limited number of appointment slots and if you cannot attend, we want to provide that slot to another patient
